Uploading Your Insulin Pump, Blood Glucose Meter, Dexcom or FreeStyle Libre
*To save time, PLEASE UPLOAD YOUR PUMP BEFORE YOUR APPOINTMENT *

 

INSULIN PUMPS

Email bloodsugars@charleshbest.com or your Diabetes Educator once you have uploaded if you would like the data to be reviewed. If you are using Libre or DexCom, follow instructions on the next page to upload this data as well.

 

If you require technical assistance, call the Technical Support number location on the back of your pump, or click on the help menu on the Diasend or Carelink website.

Medtronic Pumps & Sensors: Go to Carelink Personal https://carelink.minimed.eu

First time users: 

  • Click on “Sign Up Now” to create a Carelink Personal account

  • Login to your account and choose “Upload Device”

  • A pop up will appear and will ask you to download Uploader software.  Go to your downloads folder and open the downloaded file to install.

  • Return to Carelink Personal and click on “Upload Device”. Select “+ New Pump”, and enter the SN (serial number) of your pump (located on back of pump under the clip).

  • Continue with steps below for existing users.

Existing users:

  • Login to Carelink Personal and click on “Upload Device”.

  • Select pump. If uploading a new pump, click on “+ New Pump”, and enter the SN (serial number) of your pump (located on back of pump under the clip).

  • Step 1 - Prepare pump for upload. Clear any active alarms and finish any bolus or temp basal in progress. Disconnect pump from your body.

  • Step 2 – Under Select Link device, choose “Auto detect” from the drop down menu. Plug your Contour Next Link Blood Glucose Meter or Carelink USB into the USB port on your computer. Place pump next to your Contour Next Link Meter or Carelink USB.

  • Step 3 - Click on “Upload Now”.

  • For 630G and 670G users, a pop up will appear informing you a message will appear on your pump asking permission to connect. Click “Ok” and wait for message to appear on pump. Select “yes” on pump to connect.

  • Click “Ok” once upload complete.

NOTE: After uploading your Medtronic pump for the first time, email your username and password to your educator or bloodsugars@charleshbest.com so they may access your data.

 

Tandem T-Slim Pump: go to Diasend www.diasend.com

First time users:

  • Click “register here”. Enter your personal information to set up an account.

  • Enter clinic ID: 76-34950 to share your data.

  • Confirm registration and click “continue”.

  • Choose the Diasend Uploader to be installed on your computer (PC or Mac). Download installer, go to your download folder and click on file to install.

  • Once complete, open Diasend software from your list of programs on your computer (you may want to create a Desktop Icon).

  • Continue with steps below for existing users.

Existing time users:

  • Open Diasend on your computer.

  • Prepare pump for download. Clear any active alarms and finish any bolus or temp basal in progress. Disconnect the pump from your body.

  • Plug in USB charger cord to your pump and the other end into the USB port on your computer. Do not unplug the cable until the upload is complete.

  • Enter your email address and password, click “sign in” to view your data.

OmniPod Pump: go to Diasend www.diasend.com

First time users:

  • Click “register here”. Enter your personal information to set up an account.

  • Enter clinic ID: 03-63829 to share your data.

  • Confirm registration and click “continue”

  • Choose the Diasend Uploader to be installed on your computer (PC or Mac). Download installer, go to your download folder and click on file to install.

  • Once complete, open Diasend software from your list of programs on your computer (you may want to create a Desktop Icon).

  • Continue with steps below for existing users.

Existing time users:

  • Open Diasend on your computer.

  • Prepare pump for download. Clear any active alarms and finish any bolus or temp basal in progress.

  • Plug in USB cable to your PDM and the other end into the USB port on your computer. Do not unplug the cable until the upload is complete.

  • Enter your email address and password, click “sign in” to view your data

BLOOD GLUCOSE METERS, DEXCOM and FREESTYLE LIBRE

Email bloodsugars@charleshbest.com or your Diabetes Educator once you have uploaded if you would like the data to be reviewed. Also include your current insulin doses or to refer to your last clinic visit if no changes have been made.

 

If you require technical assistance click on “Help” from the menu on the app or website you are using. If you are having difficulty uploading data from your Libre reader or LibreLink app, call Abbott customer service at 1-800-461-8481.

Blood Glucose Meters: 

One Touch Verio Flex and Reflect Meters 

  • Download OneTouch Reveal app on your phone. (Go to App Store or Google Play and search “One Touch Reveal”. The app is a green icon with white circle).

  • Ensure Bluetooth is turned on in your phone. Go to your phone settings, select Bluetooth and check that it is set to “On”.

  • Open app and follow instructions to set up an account.

  • Go to “Pair my Meter” and follow instructions to connect your meter to the app.

  • Tap the share icon (top left corner) and under Share a Report, select “PDF”.

  • Select “Mail” to send to your educator or bloodsugars@charleshbest.com

 

Contour Meters:

  •  Download Contour app on your phone. (Go to App Store or Google Play and search “Contour”. The app is blue with a green, yellow and red dot graph).

  • Ensure Bluetooth is turned on in your phone. Go to your phone settings, select Bluetooth and check that it is set to “On”.

  • Open app and follow instructions to set up an account.

  • Go to Menu (top left) and select “Pair a meter”. Follow instructions to connect meter.

  • Go to the Menu (top left) and select “My Care”. Select “Reports”. Select the second option “Blood Sugar Diary PDF”.

  • Tap underneath “Full Name of Patient” to enter your name. Select time frame (i.e. two weeks). Select View Diary.

  • Tap the share icon (lower left corner). Select “Mail” to send to your educator or bloodsugars@charleshbest.com .

Other Meters (with USB cord):

  • Go to Diasend www.diasend.com

  • Click “register here” to create an account.

  • Enter your personal information to set up an account.

  • Enter clinic ID: 03-63829 to share your data.

  • Confirm registration and click “continue”.

  • Choose the Diasend Uploader to be installed on your computer (PC or Mac). Download installer, go to your download folder and click on file to install.

  • Once complete, open Diasend software from your list of programs on your computer (you may want to create a Desktop Icon).

  • Connect the USB cable that came with your meter (i.e. charger cord) to your computer and blood glucose meter.  Do not unplug the cable until the upload is complete.  

  • Enter your email address and password, click “sign in” to view your data.

 

 

Dexcom: go to https://clarity.dexcom.eu  

New users: 

  • Go to https://clarity.dexcom.eu and select “DexCom CLARITY for Home Users”.

  • Click on: “Create an Account” and enter your email address.

  • When you receive the email (this may take up to 10 minutes), click on “Confirm Email Address” and follow the steps to create your account.

  • Once you create your account, email bloodsugars@charleshbest.com and ask for an invitation to share your Dexcom data with our clinic.

  • You will receive e-mail from Dexcom Clarity with a link and a sharing code. Follow the link and log in with your DexCom username and password (the same log in you used to set up your DexCom account).

  • Once you are logged in, enter the sharing code from the email.

  • If you are using the DexCom receiver, you will also need to install the Clarity uploader. Select “Upload Data”. Follow instructions to download Clarity uploader file. Go to your downloads folder and click on file to run installation.

  • Continue steps below for existing users.

Existing users:

If you are using a receiver:

  • Go to https://clarity.dexcom.eu and select “DexCom CLARITY for Home Users”.

  • Log in to account.

  • Click on “upload data”. Plug your receiver into the computer using the USB cable provided. Choose upload data. 

If you are  using the Dexcom Mobile app, your data is automatically streaming to Clarity.  You only need to email when you would like the data to be reviewed 

 

FreeStyle Libre: https://www2.libreview.com/ 

 

Reader users:

  • Email your educator or bloodsugars@charleshbest.com and ask for invitation to Libreview to share your data.

  • You will receive an email inviting you to Libreview. Follow the link.

  • Select “Sign up” to create an account.

  •  ​Accept the privacy and sharing agreements. A pop up will appear asking you to accept sharing invitation from Charles. H. Best Centre, click on “Accept”. You only need to do this once.

  • Under “Upload a Device”. Click on “Download the Libreview Device Driver”. Find this file in your downloads folder, open it to install the software.

  • Return to Libreview. Plug in your reader with the yellow cord to a USB port on your computer. Click on "upload device".​

 

LibreLink App users:

  • Email your educator or bloodsugars@charleshbest.com and ask for invitation to Libreview to share your data.

  • You will receive an email inviting you to Libreview. Follow the link and log in using the email and password you used to set up the Libre app on your phone. ​

  • Accept the privacy and sharing agreements. A pop up will appear asking you to accept sharing invitation from Charles. H. Best Centre, click on “Accept”.

  • You only need to do this once. After this is completed, to have your data reviewed email bloodsugars@charleshbest.com and ask to look at your Libre data and mention that you are connected to Libreview.
