Registered Social Worker, Clinical Program

Join our interdisciplinary team as we work collaboratively to deliver clinical and mental health support to patients of all ages living with type 1 diabetes.


Part-time Position (22.5-30 hours per week)

Salary: Commensurate with experience

Schedule: Flexible weekdays and shift lengths, one evening preferred.

Location: Onsite in Whitby (HWY 407 & Thickson Rd)

-Includes the flexibility to work remotely 1 day per week



  • Formulate and deliver assessment and evidence-based treatment plans in partnership with the patient, family and interdisciplinary team to achieve progress toward goals (including management of crisis and life-threatening behaviours).

  • Conduct individual, couple, and family therapy sessions across all ages of the lifespan

  • Experience (or interest) with children and youth and family therapy is an asset

  • Develop and facilitate group psychoeducation and therapeutic sessions.

  • Ensure quality and integrity of all patient information through use of electronic medical records.

  • Work collaboratively with the interdisciplinary team to ensure continuity and efficiency of the education and support being provided to our patients and families. As an example, participate in pediatric clinics with MD, RD, and RN.



  • Master of Social Work degree 

  • Clinical and counselling experience in the assessment and treatment of children, youth and complex families in a hospital-based or community-based clinical team is an asset but novice practitioners are welcome to apply. 

  • Current (in progress) membership in good standing with the OCSWSSW

  • Desire to practice with clinical autonomy coupled with a commitment to collaborative practice

  • Experience with evidence-based treatment modalities, including Cognitive Behavioural Therapy, Dialectical Behavioural Therapy, motivational interviewing, mindfulness, crisis intervention, family therapy and group facilitation

  • Accurate, thorough and timely electronic medical record documentation skills.

  • Demonstrated commitment to ongoing and self-directed professional education and development
